No ovulation after miscarriage: what to know

After a miscarriage, the body needs time to clear pregnancy hormones and restart the communication between the brain and ovaries. Ovulation may occur before the next period, or the first cycle may be later or different from usual. A home ovulation test can be difficult to interpret while hormones are still changing, so a negative or unclear result does not explain the whole picture.

Ongoing bleeding, pain, fever, or a positive pregnancy test can point to a need for review rather than simply delayed ovulation. A clinician may use a transvaginal ultrasound or blood tests to check the uterus and hormone pattern.

Record bleeding, pain, temperature, cervical mucus, tests, and the date of the miscarriage or treatment. Avoid relying on ovulation signs for contraception until your cycle is clearer. If pregnancy is desired, ask when it is medically and emotionally appropriate to try again. If tissue remains in the uterus, treatment options can include medical management of miscarriage or surgical management of miscarriage, depending on findings and preference.

Contact a clinician if you have no returning cycle, repeated unclear tests, continuing pregnancy symptoms, or worry about recovery. Seek urgent care for heavy bleeding, severe or worsening abdominal pain, fever, fainting, dizziness, or feeling acutely unwell.
