What should you know about NICE ankylosing spondylitis?

Short answer
Ankylosing spondylitis is an inflammatory condition affecting the spine and nearby joints, often causing persistent stiffness and pain.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Main area
- Spine, sacroiliac joints and sometimes hips or ribs
- Common pattern
- Stiffness after rest that may improve with movement
- Helpful support
- Individual physiotherapy and regular gentle activity
What it means
Inflammation may be felt in the lower back, buttocks, hips or ribs. Symptoms can be worse after rest and may ease when you move. Morning joint stiffness and reduced spinal movement can make dressing, turning in bed or taking a deep breath uncomfortable. The pattern can come and go, so a symptom diary is useful.
Assessment may include an examination of movement, a review of symptoms and tests chosen by a clinician. Similar symptoms can have other causes, so a diagnosis should not be made from stiffness alone.
What you can do
Keep gently active within your limits. A physiotherapist can tailor posture, spinal mobility, breathing and strengthening work; exercise therapy is often adjusted during flares. Hydrotherapy may make movement more comfortable when land exercise feels difficult.
Take prescribed anti-inflammatory medication only as directed, and mention stomach, kidney, heart or blood-pressure concerns before using it. Alternate activity with brief rest, avoid staying in one position for long periods, and record which movements improve or aggravate symptoms.
When to seek care
Arrange a clinical review if back stiffness persists, repeatedly wakes you, limits walking or affects work and daily tasks. Seek urgent care for new loss of bladder or bowel control, numbness around the groin, marked leg weakness, or severe back pain after a significant injury. Sudden eye pain, light sensitivity or blurred vision also needs prompt medical attention.
Questions for your doctor
Ask which findings support the diagnosis, whether another condition should be excluded, and what activity level is suitable during a flare. You can also ask how progress will be monitored and when medication review is needed.
