What should you know about a nerve lump on the neck?

A lump on the neck near a nerve needs clinical assessment because its cause can range from an inflamed node to a tumour. The word nerve describes location or suspected origin; it does not by itself identify cancer. A clinician will examine the lump and the surrounding neck, mouth, throat and thyroid area.

Neck lumps may be enlarged lymph nodes, thyroid nodules, salivary-gland changes, cysts, infections or growths arising from nerve tissue. A lump can be tender during infection, while a firm, fixed or gradually enlarging lump deserves prompt review. A neck lump is a possible presentation of head and neck cancer, lymphoma or thyroid cancer, but the lump alone cannot establish any of these diagnoses. If tests find cancer but its starting point is unclear, clinicians may investigate cancer of unknown primary.

Arrange an appointment with a doctor or ear, nose and throat specialist, especially if the lump persists, grows, returns or is difficult to move. Assessment may include a history, examination, ultrasound or other imaging, and a needle sample or biopsy when needed. Do not repeatedly press or massage the area. Record when you first noticed it, any change in size, pain, voice change, swallowing difficulty, weight change or recent infection. Treatment depends on the confirmed cause; thyroid-related findings may lead to a discussion of thyroid removal surgery.

Seek urgent medical help if the swelling makes breathing or swallowing difficult, rapidly increases, causes severe pain, or is accompanied by a muffled voice, drooling, marked weakness or a high fever. Arrange prompt rather than routine review for a hard or fixed lump, unexplained bleeding, persistent hoarseness or a lump that does not settle after an infection has improved.

Ask what structures may be involved, which examination or scan is appropriate, whether a tissue sample is needed, and which changes should prompt faster review. Ask when you should expect results and what the next step would be for an infection, thyroid finding, lymph-node change or nerve-related growth.
