Neonatal Chickenpox: What to Know

Chickenpox is caused by the varicella-zoster virus. It often starts with unsettled behaviour, reduced feeding or fever, followed by an itchy rash that becomes fluid-filled blisters. In a newborn, the pattern can change quickly, and the rash may be difficult to distinguish from other infections.

Tell the clinician if the baby was exposed to someone with chickenpox or shingles, and whether the birth parent developed a similar rash around delivery. Shingles comes from the same virus family, but a newborn’s risk depends on the type and timing of exposure.

Contact the baby’s maternity, paediatric or urgent medical service promptly rather than waiting for the rash to spread. Keep the baby away from pregnant people, newborns and anyone with weakened immunity until a clinician gives clear advice. Do not apply creams, give medicines or open blisters without guidance.

Keep a note of feeding, wet nappies, temperature and the timing of new blisters. A clinician may consider Antiviral Treatment or other monitoring based on the baby’s age, exposure and examination. A GP Consultation may be appropriate only if the service advises it for this newborn.

Get emergency help if the baby is difficult to wake, breathing unusually, feeding poorly, vomiting repeatedly, has fewer wet nappies, develops a seizure, or has a rapidly spreading blistering rash. Fever, marked sleepiness or a baby who looks unwell also warrants urgent assessment.
