Miscarriage after implantation: what to know

After implantation, light spotting may occur and does not by itself establish pregnancy loss. Increasing bleeding, pelvic cramps, or pregnancy symptoms changing can occur with miscarriage, but they may also occur in other situations. Pain or bleeding can also occur with an ectopic pregnancy, so pregnancy location matters.

A clinician may use serial pregnancy hormone measurements and a transvaginal ultrasound. Very early in pregnancy, one scan may not answer every question, and a repeat assessment may be needed. Avoid interpreting a home test as proof that the pregnancy is continuing or has ended.

Contact a gynaecology or early pregnancy service, describe the amount and colour of bleeding, any clots, pain location, and your last test result. Use sanitary pads while bleeding and avoid inserting anything into the vagina until a clinician advises otherwise. Keep hydrated and arrange support if you feel distressed or weak.

If miscarriage is confirmed, your clinician can discuss expectant care, medical management, or surgical management. The suitable choice depends on bleeding, infection, scan findings, medical history, and your preference.

Go to urgent care for severe or one-sided pain, fainting, shoulder-tip pain, fever, feeling very unwell, or rapidly heavy bleeding. These symptoms may reflect ectopic pregnancy, infection, or significant blood loss and need prompt assessment.

Ask when to repeat pregnancy hormone testing, whether a transvaginal ultrasound is appropriate now, how the pregnancy location will be checked, and what treatment choices would be available if miscarriage is confirmed.
