What should you know about mild croup cough?

A mild croup cough is usually a harsh, barking cough caused by swelling around a child’s upper airway, often during a viral illness. It may sound worse at night and can occur with a hoarse voice, runny nose, or low fever. A child with mild croup is generally comfortable between coughing episodes and breathes quietly when resting. The cough itself can be unsettling, but the key concern is how easily the child is breathing.

Croup affects the voice box and nearby upper airway. Narrowing in this area creates the distinctive barking sound; crying or agitation can make the noise and breathing effort more noticeable. Mild illness usually has no noisy breathing while the child is calm and resting. Croup commonly starts after cold-like symptoms and may improve over several days, although the cough can linger for a little longer. Other cough causes, including a common cold, acute bronchitis, or bronchiolitis, can sound different and may need different assessment.

Keep your child calm and hold them upright if that feels comfortable. Offer frequent small drinks to reduce the chance of dehydration, and allow rest. Check on breathing regularly, especially during sleep or when the cough suddenly worsens. A cool, comfortable room may help, but avoid steam or placing a child near hot water because of burn risk. Do not give cough or cold medicines to a young child unless a clinician or pharmacist has advised that the specific product is suitable. A clinician may prescribe medicine that reduces airway swelling when symptoms warrant it; antibiotics do not treat the viral causes of croup.

Seek urgent medical help if your child has noisy, high-pitched breathing while resting, is working hard to breathe, or has skin pulling in between or below the ribs. Get emergency help if the lips or face look blue or grey, the child is unusually sleepy or difficult to wake, cannot swallow, is drooling, or cannot speak or cry normally because of breathlessness. Arrange prompt clinical advice if drinking falls, urination becomes noticeably less frequent, fever concerns you, symptoms are worsening, or the cough is not settling. Keep the child upright and calm while help is arranged.
