What happens in the forefoot

A bursa cushions movement between tissues. Repeated pressure from narrow footwear, a changed walking pattern, high-impact activity, or an altered forefoot shape can irritate one near the metatarsal heads. Pain may feel burning, aching, or like a small pebble under the front of the foot. The area can be tender when pressed and may swell. Similar symptoms can arise from a stress injury, nerve irritation, skin problems, or a joint disorder, so examination matters.

Ways to reduce pressure

Choose footwear with enough room for the toes and a sole that reduces forefoot pressure. Temporarily reduce activities that make walking painful, and use a gradual return rather than pushing through worsening symptoms. A clinician may suggest a pad or insole to redistribute pressure. Anti-Inflammatory Medication may be appropriate for some people after a safety review. If symptoms persist, a Cortisone Injection or Joint Aspiration may be discussed, depending on the swelling and diagnosis.

When to seek care

Arrange an assessment if swelling, warmth, or pain continues despite reducing pressure, or if walking becomes increasingly difficult. Seek urgent care for rapidly spreading redness, feverish symptoms, drainage, a wound, severe pain after an injury, or a cold or numb foot. People with diabetes, poor circulation, or reduced sensation should seek advice early when a foot becomes swollen or sore.
