Short answer

Possible miscarriage symptoms include vaginal bleeding, pelvic cramping, or passing tissue, but these signs need medical assessment rather than self-diagnosis.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Possible symptoms
Bleeding, pelvic cramps, backache, clots, or tissue passage.
Assessment
A clinician may use examination, blood tests, and transvaginal ultrasound.
Urgent signs
Severe pain, fainting, heavy bleeding, fever, or feeling very unwell.

What the symptoms can mean

Early pregnancy bleeding may be light or heavy and can occur with cramps, backache, clots, or tissue. These findings can occur with miscarriage, but bleeding does not by itself confirm pregnancy loss. A pregnancy outside the womb and other causes of pain or bleeding also need consideration. The clinician may ask about your last menstrual period, symptoms, blood group, and previous scans.

A transvaginal ultrasound can help assess the pregnancy and uterus. Sometimes the first assessment cannot provide a definite answer, so repeat examination or testing may be discussed. This uncertainty can be emotionally difficult; it is reasonable to ask what is known now and what will clarify the situation.

What to do next

Contact your maternity or gynaecology service for advice about any bleeding or pain in pregnancy. Note when symptoms began, how they change, and whether you pass clots or tissue. Use a pad rather than a tampon so bleeding can be observed, and avoid inserting anything into the vagina until a clinician advises you. Do not take medicines for pain without checking which options are suitable for you.

When to seek urgent care

Seek emergency help for severe or one-sided abdominal pain, shoulder-tip pain, fainting, dizziness, heavy bleeding, fever, or feeling very unwell. These symptoms can indicate a complication that needs rapid treatment. If a miscarriage is confirmed, your clinician may explain expectant care or options such as Medical Management of Miscarriage and Surgical Management of Miscarriage.

Questions for your doctor

Ask what your examination and ultrasound show, whether repeat testing is needed, and which symptoms should bring you back urgently. If loss is confirmed, ask how each management option works, how pain and bleeding will be handled, and when follow-up is planned.

Questions people ask

Does bleeding always mean miscarriage?

No. Bleeding can have several causes, so an examination and appropriate testing are needed before drawing conclusions.

What if the first ultrasound is unclear?

Your clinician may recommend repeat testing or monitoring because an early scan may not yet show enough information.
