What it means
A bursa cushions tissues where they move against one another. Repeated friction, pressure from sitting or clothing, a blow to the hip area, or changes in activity can irritate the bursa near the iliac crest. Pain may be felt along the top of the hip or outer pelvis and can be more noticeable when lying on that side, walking, climbing, or pressing the area. Visible swelling is not always present; when it is, it may fit the broader pattern of joint swelling but can also have other causes.
A clinician may examine the tender area, hip movement, back, and nearby muscles. Imaging or fluid testing may be considered when the diagnosis is uncertain, symptoms persist, or infection is a concern.

Reduce the activity or pressure that clearly provokes the pain for a short period, while keeping gentle movement within comfort. Avoid repeatedly testing the sore spot. Ice wrapped in a cloth may help after aggravating activity. A clinician or pharmacist can advise whether anti-inflammatory medication is suitable for you, especially if you have stomach, kidney, heart, pregnancy, or medication concerns. Persistent symptoms may lead to discussion of a cortisone injection; aspiration is reserved for selected cases where fluid needs assessment or removal.
When to seek care
Arrange an assessment if pain is worsening, limits walking or sleep, returns whenever normal activity resumes, or does not settle with simple changes. Seek prompt medical care for fever, spreading redness, warmth, marked swelling, drainage, or feeling acutely unwell, because these features can signal infection. After a significant fall or blow, urgent evaluation is sensible when you cannot bear weight or pain is severe.
