What the tiredness may mean

Pain, bloating, diarrhoea or constipation can make rest and normal activities harder. Worry about symptoms, frequent bathroom trips, and waking during the night may add to daytime fatigue. Eating less, avoiding several food groups, or replacing meals because you fear symptoms can also leave you short of energy or nutrients.

Fatigue is not specific to IBS. A clinician may consider sleep problems, low mood, medication effects, infection, or another condition. Persistent tiredness with weight loss, blood in the stool, fever, or a marked change in bowel habits should not simply be attributed to IBS. Anaemia, coeliac disease, dehydration, and chronic kidney disease are examples of other problems that may need checking.

Keep a brief diary of sleep, meals, bowel symptoms, pain, stress, and energy. This can reveal patterns without requiring you to remove foods at random. Aim for regular meals, adequate fluids, gentle movement, and a consistent sleep routine. If food avoidance is growing, ask a doctor or dietitian for guidance before making major dietary changes.

Treating the IBS symptoms that interrupt sleep or daily life may improve how you feel, but treatment should match your bowel pattern and medical history. Do not use laxatives, anti-diarrhoeal medicines, or supplements regularly without checking that they are suitable for you.

When to seek care

Arrange a medical appointment if tiredness lasts, is worsening, limits work or daily activities, or does not improve as your bowel symptoms settle. Seek prompt care for black or bloody stools, unexplained weight loss, persistent vomiting, fever, severe or steadily worsening abdominal pain, fainting, or signs of dehydration such as very little urine and dizziness.

Questions for your doctor

Ask whether your fatigue could be related to sleep disruption, diet, medicines, or a separate health problem; whether you need tests for anaemia or coeliac disease; and which IBS treatment is appropriate for your bowel symptoms. Mention any bleeding, weight change, fever, night waking, or foods you have stopped eating.
