What chest tightness can mean

Chest tightness may feel like pressure, squeezing, or difficulty taking a full breath. Asthma and COPD can narrow the airways and often bring wheezing, cough, or breathlessness. Acid reflux may cause burning or pressure behind the breastbone. Anxiety or panic can tighten chest muscles and change breathing. Angina is chest discomfort caused by reduced blood flow to heart muscle and may appear with exertion, stress, or breathlessness. The sensation alone cannot distinguish these causes.

What to do now

Stop strenuous activity and sit upright while you assess what is happening. Note when the tightness began, what triggers it, how long it lasts, and whether it spreads to the arm, jaw, back, or shoulder. Use a prescribed reliever inhaler only as directed; do not borrow someone else’s inhaler. Arrange medical review for recurring, unexplained, or activity-related episodes, even if they settle.

When to seek emergency care

Call emergency services for severe or persistent tightness, pressure with sweating or nausea, pain spreading to the arm or jaw, faintness, blue lips, sudden breathlessness, or confusion. Treat sudden symptoms as urgent rather than assuming they are anxiety, reflux, or asthma. Do not drive yourself if you feel faint or struggle to breathe.
