Hysterectomy after three caesarean births is possible, but previous surgical scarring can make planning and bladder protection especially important. Scar tissue may join the womb to nearby structures, so the surgeon chooses the safest route after reviewing your examination, imaging, symptoms, and reason for surgery. The number of caesareans alone does not determine whether hysterectomy is suitable.

A hysterectomy removes the womb. Depending on the indication, the cervix, fallopian tubes, or ovaries may also be removed; these are separate decisions to discuss. Previous caesarean scars can alter the normal tissue planes around the bladder and womb. This may affect the operation time, surgical route, and possibility of needing additional specialist support. Hysterectomy may be considered for fibroids, adenomyosis, endometriosis, heavy periods, pelvic pain, abnormal womb lining, or cancer-related care.

Your surgeon should explain whether the operation is abdominal, vaginal, or minimally invasive, what will be removed, and how fertility and menopause symptoms may be affected.

Ask for a clear explanation of the diagnosis and alternatives before consenting. Share details of every caesarean, prior complication, medicine, allergy, and abdominal operation. Take the opportunity to discuss blood tests, imaging, anaesthesia, bladder or bowel considerations, expected hospital care, and activity restrictions. If the ovaries remain, they may continue producing hormones; removing them can bring on surgical menopause, so the plan should be explicit.

Contact your surgical team promptly for worsening pain, fever, increasing redness or discharge from an incision, difficulty passing urine, heavy vaginal bleeding, foul-smelling discharge, calf swelling, chest pain, or breathlessness. Emergency care is needed for severe breathlessness, collapse, or sudden chest pain. Follow the discharge plan even if your wound looks normal, because internal healing also needs time.
