What should you know about hyperpigmentation in lips?

Short answer

Lip hyperpigmentation can follow sun exposure, dryness, irritation, inflammation, smoking, or a medicine-related reaction.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Frequent triggers
Sun, dryness, licking, cosmetics, and irritation
First measures
Bland moisturiser and consistent lip sun protection
Do not do
Apply unprescribed acids or bleaching products to lips

What it can mean

Lips have delicate, visible skin, so repeated licking, biting, fragranced balms, lipstick, or dental products can cause irritation followed by darker colour. Sun exposure may deepen existing pigment, especially when lip protection is missing. Darkening can affect the whole lip or appear as a separate brown, grey, or black mark. A clinician should inspect a new isolated spot, particularly if it changes, bleeds, crusts, or fails to heal. The appearance alone cannot identify its cause.

What you can do

Use a bland moisturising ointment, avoid licking or picking, and stop any product that burns or causes peeling. Choose a lip product with sun protection and reapply when outdoors. Do not place facial bleaching creams, acids, or salon peels on the lips without specialist guidance. A dermatologist may consider Skin Brightening Treatment, a carefully selected Chemical Peel, or another approach after identifying the pigment’s cause. IPL Photofacial, Picosecond Laser Treatment, or Hydroquinone Treatment may not suit every lip area and should never be self-prescribed.

When to seek care

Book a dermatology review for a new or enlarging dark mark, uneven pigment that keeps changing, persistent soreness, bleeding, crusting, ulceration, or a non-healing patch. Seek care sooner if swelling or blistering follows a product or treatment. A changing spot needs examination rather than cosmetic treatment first.

Questions for your doctor

Ask whether the colour is caused by irritation, sun, medication, or a separate lesion. Ask which lip products are suitable, whether testing is needed, and how to protect the area during any treatment.

Yes. Repeated dryness and irritation can be followed by post-inflammatory pigment.

Is every dark lip spot harmless?

No. A new or changing isolated spot should be examined before cosmetic treatment.
