What should you know about hyperadrenergic orthostatic hypotension?

Short answer
Hyperadrenergic orthostatic hypotension causes symptoms after standing because blood pressure regulation is impaired and the body responds with increased adrenaline activity.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance

- Symptoms commonly appear after standing

- Sit or lie down promptly when symptoms start
What it means
When you stand, blood temporarily shifts toward the legs. The nervous system normally adjusts circulation to maintain blood flow to the brain. With orthostatic hypotension, that adjustment is insufficient, and the adrenergic response may produce a racing or forceful heartbeat, tremor, sweating, anxiety-like sensations, or a feeling of internal alarm. Dizziness, blurred vision, weakness, or near-fainting may follow. Several health conditions and medicines can contribute.
What you can do
Rise in stages: sit first, move your feet, then stand while holding a stable surface. If symptoms begin, sit or lie down promptly and avoid driving until the pattern is understood. Keep hydrated unless a clinician has restricted fluids, and note posture, meals, heat, exertion, medicines, heartbeat sensations, and recovery time. A GP Consultation can check lying and standing measurements and review possible causes.
When to seek care
Seek urgent help for fainting with injury, chest pain, severe breathlessness, a new irregular heartbeat, confusion, one-sided weakness, or symptoms that do not settle after lying down. Arrange prompt assessment for repeated near-fainting, falls, worsening exercise intolerance, or a new problem after starting or changing a medicine. Do not alter prescribed treatment without advice.
Questions for your doctor
Could a medicine, dehydration, anaemia, or heart rhythm problem be contributing? Should I record pulse and blood pressure in different positions? Which fluids, activity changes, or compression measures are suitable for me?

Questions people ask
Why can standing cause a racing heartbeat?
The body may increase adrenaline activity to compensate for reduced circulation after standing, producing a racing or forceful heartbeat.
Should I ignore occasional dizziness on standing?
Repeated, severe, or worsening episodes should be assessed, particularly if they cause near-fainting, falls, chest symptoms, or injury.
