Hydrosalpinx sonography: what the scan can show

Hydrosalpinx sonography uses ultrasound, usually a transvaginal scan, to look for a fallopian tube that is widened and filled with fluid. The scan does not use radiation. A sonographer or clinician may examine the uterus, ovaries and areas beside the uterus, then describe the size, shape and appearance of any fluid-filled structure. An ultrasound appearance can suggest hydrosalpinx, but the result is interpreted alongside your symptoms, examination and fertility plans.

A hydrosalpinx is a fallopian tube that has become blocked and distended with fluid. On ultrasound, it may appear as an elongated or folded, fluid-filled structure near an ovary or the uterus. Thin internal walls, incomplete partitions, or small rounded projections can help distinguish a tube from a simple ovarian cyst, although appearances can overlap. A prior pelvic infection, including pelvic inflammatory disease, is one possible cause; previous surgery, endometriosis or other inflammation may also contribute.

Some people have no symptoms. Others experience pelvic discomfort, pain during sex, unusual discharge or difficulty becoming pregnant. A hydrosalpinx can affect the movement of sperm or an embryo through the tube. If pregnancy is being planned, your clinician may discuss whether the finding could affect fertility treatment or increase concern about a pregnancy developing outside the uterus.

Arrange a review of the report with a gynaecologist or fertility specialist, particularly if the scan was performed during an infertility assessment. They may compare it with earlier imaging and decide whether further assessment is useful. A standard ultrasound cannot always show whether a tube is open; a separate test may be suggested when tubal openness matters.

Treatment depends on symptoms, the appearance of the tube, infection concerns and whether you are trying to conceive. Options may include observation when appropriate, treatment for an active infection, or surgery such as salpingectomy. For some fertility plans, the team may discuss IVF and how a hydrosalpinx could affect the treatment approach. Do not start antibiotics without a clinician’s advice, because the scan alone does not prove an active infection.

Seek urgent medical help for sudden or worsening one-sided pelvic pain, fainting, marked dizziness, fever with pelvic pain, repeated vomiting, or heavy vaginal bleeding. If you could be pregnant and have pelvic pain or bleeding, seek prompt assessment even if an earlier scan mentioned hydrosalpinx. These symptoms need evaluation because several urgent pelvic and pregnancy-related conditions can look similar at first.

Ask whether the scan clearly showed a fallopian tube, whether one or both tubes appear affected, and whether the finding could be confused with an ovarian cyst or another pelvic structure. You can also ask if you need a test of tubal openness, whether infection needs assessment, and how the result changes the timing or choice of fertility treatment.
