Hook wire biopsy

When an abnormal area cannot be easily felt, a radiologist uses mammography or ultrasound to position a fine wire with its hooked tip beside the area of concern. The wire marks the location while the patient travels to surgery, where the surgeon follows it to remove the target and a margin of surrounding tissue when appropriate. The specimen goes to a laboratory. A biopsy can show whether cells are benign, precancerous or cancerous, and laboratory methods such as immunohistochemistry may provide additional detail. The wire is a guide, not a treatment and not a diagnosis by itself.

Follow the centre's instructions about food, medicines and arrival time. Tell the team about blood thinners, allergies, pregnancy possibility and previous reactions to local anaesthetic. During placement, you may feel pressure or a brief sharp sensation; local anaesthetic is commonly used. Keep the dressing clean and follow wound-care advice after surgery. Mild bruising or soreness can occur. Use recommended pain relief, avoid strenuous activity until advised, and ask when the pathology result will be reviewed.

Contact the surgical team for increasing redness, warmth, swelling, drainage, fever, worsening pain, persistent bleeding or a dressing that becomes soaked. Seek urgent help for heavy bleeding, breathing difficulty, fainting or sudden severe swelling. A missing or displaced wire before surgery should be reported immediately so the imaging and surgical teams can decide the next step. The result may require further appointments; waiting for it does not mean that cancer has been confirmed.
