Understanding a pulled hamstring

The hamstrings are muscles at the back of the thigh. A strain can happen when they lengthen and contract suddenly during sprinting, jumping, or a slip. Pain may be felt in the back of the thigh or near the buttock, with tenderness, tightness, bruising, or weakness. A mild strain can improve with careful activity changes, while a larger tear may need structured rehabilitation. Thigh pain can have other causes, so the mechanism and examination matter.

Home care and gradual recovery

Stop the activity that caused pain and walk only as comfortably as you can. Apply a cold pack wrapped in cloth for short sessions during the early phase. Rest the leg in a comfortable position and avoid deep massage, heat, sprinting, and aggressive stretching at first. As ordinary walking becomes easier, add gentle range-of-motion work, followed by controlled strengthening guided by symptoms. Return to running only when strength and movement are comparable on both sides and faster activity does not bring pain back. Exercise therapy or sports physiotherapy can provide a staged plan.

When an assessment is needed

Seek prompt care if you heard a pop, cannot walk normally, have extensive bruising or swelling, notice a visible gap, or develop marked weakness. Get assessed if pain is high near the sitting bone, symptoms worsen, or improvement stalls. A clinician can distinguish a strain from another injury and decide whether rehabilitation, imaging, or a treatment such as PRP injection is appropriate.
