What it can mean
Viral illnesses such as bronchiolitis or a common cold may cause cough and congestion. A high-pitched sound can also occur when the upper airway is irritated or narrowed. Reflux, asthma, allergic rhinitis, acute bronchitis, and less common airway or lung conditions are other possibilities. The sound alone cannot identify the cause.
Watch the infant’s breathing when calm: look for ribs pulling in, nostril flaring, grunting, pauses, or noisy breathing. Note feeding amounts, wet nappies, fever, vomiting, sleepiness, and whether the cough began suddenly during feeding or after a possible choking episode.
What to do now
Keep the infant upright while awake, clear visible nasal mucus gently, and offer usual feeds in smaller, more frequent amounts if feeding is tiring. Keep smoke, aerosols, and fragrances away. Do not give honey, over-the-counter cough medicine, or another person’s medicine to an infant.
Contact a clinician promptly for a new high-pitched cough, reduced feeding, fewer wet nappies, fever, repeated vomiting, or a cough that is worsening. Record a short description of the sound and when it occurs if doing so does not delay care.
When to seek urgent care
Call emergency services if the infant is struggling to breathe, has pauses, blue or grey lips, severe chest indrawing, cannot feed because of breathlessness, becomes unusually floppy or difficult to wake, or develops a sudden cough after choking. Do not wait for the cough to settle in these situations.
