Why bleeding may change

During perimenopause, ovulation can become less regular. Hormone fluctuations may make periods closer together, farther apart, longer, or heavier, sometimes with spotting between cycles. Those changes are common in this transition, but they do not explain every episode of bleeding. Fibroids, polyps, adenomyosis, endometrial overgrowth, thyroid problems, medicines, and pregnancy can also affect flow. A heavy period is a symptom, not proof of perimenopause or another condition. Repeated blood loss may reduce iron stores and cause tiredness, headaches, reduced exercise tolerance, or breathlessness.

Planning an appointment

Record cycle dates, the number of heavy days, spotting, clots, pain, hot flushes, medicines, and contraception. Note whether bleeding occurs after sex or after a long gap without a period. Arrange a clinical review for a new or changing pattern, even if other perimenopause symptoms are present. Assessment may include a pregnancy test, blood count, iron testing, pelvic examination, ultrasound, or other testing chosen from your history. Treatment depends on the cause, bleeding pattern, migraine and clotting history, blood pressure, and whether contraception is still needed.

Red flags

Seek urgent care for rapidly soaking bleeding, fainting, severe dizziness, chest pain, shortness of breath at rest, severe pelvic pain, or a positive pregnancy test with pain or bleeding. Arrange prompt review for bleeding after sex, bleeding after twelve months without a period, or a pattern that repeatedly interrupts daily life. Do not assume that every change is hormonal; the timing and accompanying symptoms guide the urgency.
