Heavy clotting during a period

Heavy clotting during a period may reflect heavy menstrual bleeding, especially when flow is rapid, and recurring changes should be checked. Clots form when blood and uterine lining leave the uterus faster than the body can keep them fluid. An isolated clot can be less concerning than a pattern that is new, increasingly heavy, or disruptive.

Heavy periods can present with clots, but clots alone cannot reveal the cause. Uterine fibroids, adenomyosis, endometrial polyps, endometrial hyperplasia, perimenopause, and polycystic ovary syndrome can all be relevant possibilities. Blood loss may also contribute to iron deficiency anaemia, with symptoms such as low energy, dizziness, headaches, or breathlessness. Record whether clots occur with pelvic pressure, painful cramps, bleeding between periods, or an irregular cycle.

Write down the first and last day of bleeding, the heaviest part of the cycle, changes in period products, clot appearance, pain, and missed activities. Book a medical review when heavy clotting keeps returning or affects work, sleep, exercise, or intimacy. Your clinician may review medicines and contraception, check for pregnancy where relevant, and consider a pelvic examination, blood tests, or ultrasound. Treatment depends on the cause and your plans for pregnancy.

Seek urgent care for heavy bleeding with faintness, marked weakness, shortness of breath, severe pain, or confusion. Seek prompt assessment if pregnancy is possible, if fever accompanies pelvic symptoms, or if bleeding is rapidly becoming heavier. These warning signs call for timely evaluation; they do not depend on whether clots are common for you.

Ask what may be driving the change, whether anaemia testing is suitable, whether imaging could help, and which options might reduce bleeding. Also ask how to adjust treatment if you are trying to conceive, using hormonal contraception, or taking blood-thinning medicine.
