Heavy bleeding after sex for the first time: what to know

A small amount of spotting can happen when delicate tissue at the vaginal opening or inside the vagina is stretched or irritated. Heavy flow, bleeding that does not settle, or bleeding that returns after sex has other possible explanations and should not be assumed to be caused by first-time sex alone.

Friction, limited lubrication, or a small tear can cause bleeding during or soon after penetration. Bleeding from the cervix may occur with inflammation, an infection such as Chlamydia, or changes in cervical cells such as Cervical Dysplasia. Vaginal dryness related to Genitourinary Syndrome of Menopause can also make tissue more fragile.

Less common but important causes include abnormal changes of the cervix or vagina, including Cervical Cancer or Vaginal Cancer. A bleeding episode does not diagnose any of these conditions; a clinician may need to examine the vagina and cervix and decide whether testing is appropriate.

Stop sexual activity until the bleeding has stopped and you have guidance if it was heavy or unusual. Use a pad so you can observe whether the flow is easing, and note when it began, whether clots or pain occurred, and whether pregnancy is possible. Avoid putting anything in the vagina while bleeding is active.

Arrange a prompt appointment with a gynaecology or primary-care clinician. They may ask about your menstrual cycle, contraception, pregnancy possibility, infection symptoms, and medicines, then offer an examination or tests based on your history. Do not douche or use leftover treatments before assessment.

Seek urgent medical care if the bleeding is rapidly soaking pads, you feel faint or unusually weak, or you have severe or worsening pelvic or abdominal pain. Urgent assessment is also needed for fever, shoulder-tip pain, or bleeding with a possible pregnancy.

If the bleeding has stopped but was clearly heavier than spotting, contact a clinician soon. Emergency services are appropriate if you feel you may collapse, have trouble breathing, or cannot safely travel for care.

Ask what the examination suggests, whether you need pregnancy or infection testing, and whether cervical screening or follow-up is due. You can also ask which symptoms should prompt immediate return, when it is sensible to resume sex, and whether a tear or dryness needs specific care.
