Heavy aching legs and fatigue: possible causes and next steps

Short answer
Heavy aching legs with fatigue can follow overuse, poor sleep, fluid loss or an underlying condition, so the pattern and accompanying symptoms matter.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What the combination may reflect
Leg heaviness can come from tired muscles, prolonged standing, reduced activity or swelling. Fatigue may accompany a viral illness, inadequate sleep, emotional strain or low fluid intake. Anaemia can present with fatigue and reduced exercise tolerance; Chronic Kidney Disease, Coeliac Disease, COVID-19, Burnout, Alcohol Use Disorder and Chronic Fatigue Syndrome are other conditions a clinician may consider when the history fits. These possibilities cannot be sorted from symptoms alone.
Track the pattern and support recovery
Note when symptoms began, whether one or both legs are affected, what makes them better, and whether swelling, breathlessness, fever, weight change or bowel symptoms occur. Pace activity, take gentle movement breaks, sleep consistently and drink fluids unless you have been given a fluid restriction. Avoid suddenly increasing exercise. If symptoms started after vomiting, diarrhoea, heat exposure or poor intake, read about [Dehydration](/conditions/dehydration/). Book a routine appointment if fatigue persists, limits daily activities or keeps returning; a clinician can review medicines and decide whether examination or blood tests are useful.
Warning signs
Seek urgent help for one-sided leg swelling with warmth or pain, sudden weakness, chest pain, severe breathlessness, fainting, confusion or new difficulty walking. Prompt assessment is also needed for black stools, heavy bleeding, persistent vomiting or rapidly worsening symptoms. Do not massage a suddenly swollen painful leg until it has been assessed.
Questions for your appointment
Ask which causes fit your timing and other symptoms, whether your medicines or sleep need review, and what tests could check blood count, thyroid function, kidney health or nutrition. Ask how to exercise safely while energy is low.

Questions people ask
Does fatigue with aching legs identify one condition?
No. Several physical, lifestyle and emotional factors can overlap, so an assessment depends on the full pattern.
Should I rest completely?
Usually gentle movement with sensible pacing is preferable to prolonged inactivity, unless a clinician advises otherwise.
