Heart palpitations and Zoloft: what should you know?

Palpitations can feel like pounding, fluttering, skipped beats, or a fast pulse. Anxiety, poor sleep, dehydration, fever, caffeine, nicotine, or another medicine can contribute. A clinician may also consider anaemia, thyroid overactivity, low blood sugar, or an abnormal rhythm such as atrial fibrillation. The timing matters: note whether symptoms began after starting Zoloft, after a dose change, or during a stressful moment. A medication review can separate a possible treatment effect from another cause.

Sit down when an episode starts and breathe slowly. Avoid extra caffeine, nicotine, and recreational stimulants until you have spoken with a clinician. Keep taking Zoloft exactly as prescribed unless the prescriber tells you otherwise. Write down the time, duration, pulse if you can measure it, dose timing, food, drinks, and associated symptoms. Arrange a medication review if episodes recur, last longer, or interfere with sleep or activity. The assessment may include an examination, an electrocardiogram, and blood tests guided by your symptoms.

Seek emergency help for palpitations with chest pressure, severe breathlessness, fainting, confusion, or a sustained very fast or irregular heartbeat. Get prompt medical advice if you have new swelling, unusual tiredness, or repeated episodes. These symptoms can occur with problems such as heart failure, atrial fibrillation, anaemia, or hyperthyroidism and need assessment rather than self-diagnosis.

Ask whether the timing fits a dose effect, whether another medicine or supplement could interact, and whether you need an ECG or blood tests. Ask how to taper safely if stopping is considered, and what symptom should prompt urgent help.
