Heart fatigue: possible meanings and next steps

Some people use heart fatigue to describe low energy with exertion, breathlessness, awareness of the heartbeat or reduced ability to do usual activities. These features can occur with heart problems, but tiredness alone does not identify its cause.

Other explanations include anaemia, dehydration, a recent viral illness such as COVID-19, burnout, sleep problems, thyroid disease, kidney disease, coeliac disease or alcohol-related problems. The pattern matters: note when fatigue began, what worsens it, and whether rest helps.

Book a primary-care assessment if fatigue persists, interferes with work or daily tasks, or comes with breathlessness, swelling, dizziness, fainting, chest discomfort or palpitations. Your clinician may ask about sleep, mood, diet, bleeding, medicines and recent infection, then decide which examinations or blood tests are appropriate.

Until reviewed, pace demanding activities, maintain regular meals and fluids, and keep a simple symptom diary. Avoid using energy drinks or supplements as a substitute for finding the cause, particularly if your heartbeat feels unusual.

Get emergency help for chest pressure, severe breathlessness, fainting, sudden confusion, blue lips or a new rapid heartbeat with weakness. Do not drive yourself if you feel unsafe or might collapse.

Seek prompt advice if fatigue is rapidly worsening, follows repeated vomiting or diarrhoea, or occurs with black stools, unusual bleeding, ankle swelling or breathlessness when lying down.

Ask which causes fit your symptom pattern, whether you need blood tests for anaemia, thyroid or kidney problems, and whether your medicines, sleep or recent illness could be contributing.
