Symptoms of heart failure with preserved ejection fraction

In this form of heart failure, the main pumping chamber may be less flexible and may not fill easily. Pressure can then rise in the heart and lungs. Many people notice shortness of breath during walking or stairs, tiredness after ordinary tasks, or reduced exercise tolerance. A cough may be worse when lying down. Fluid can collect around the ankles, and palpitations may reflect a rhythm problem. Symptoms can build slowly, and a preserved ejection fraction does not exclude heart failure. Similar symptoms also occur with lung disease, anaemia, rhythm disorders, and other conditions.

Book an assessment for persistent or changing breathlessness, fatigue, cough, palpitations, or swelling. Note triggers, sleeping position, activity tolerance and whether symptoms settle with rest. Bring your medicine list and mention Hypertension, kidney disease, diabetes, or Atrial Fibrillation if relevant. Assessment may include blood pressure and rhythm checks, blood tests, an electrocardiogram, chest evaluation, and echocardiography. Follow the agreed plan for medicines, activity and fluid management; do not alter prescriptions independently.

Call emergency services for severe breathlessness, chest pain, fainting, collapse, sudden confusion, or blue or grey lips. Seek prompt advice for quickly worsening swelling, breathlessness when resting or lying down, or palpitations accompanied by dizziness, weakness, or near-fainting. Sit upright and keep your medication information available while help is arranged.
