What swollen feet can mean

Heart failure can raise pressure in the veins, allowing fluid to move into nearby tissues. This often causes swelling around the ankles, feet, and lower legs. The skin may feel tight, shoes may become uncomfortable, and a dent can remain briefly after pressing the swollen area. Swelling may be more noticeable later in the day or after standing.

Check both feet and ankles for changes and note when the swelling appears, whether it is spreading, and whether your breathing or exercise tolerance has changed. Contact your heart-failure team or a clinician if the swelling is new, increasing, or accompanied by rapid change in your usual weight, worsening fatigue, cough, or breathlessness.

When to seek care

Seek emergency help now for severe or sudden breathlessness, chest pain or pressure, fainting, confusion, blue or grey lips, or coughing up pink frothy sputum. Sit upright while waiting and do not drive yourself if you feel seriously unwell.

Arrange urgent medical advice for swelling that worsens quickly, spreads up the legs, affects one leg with pain or warmth, or occurs with breathlessness at rest, a new cough, marked tiredness, or a racing or irregular heartbeat. A clinician may review your symptoms, medicines, blood pressure, heart rhythm, and signs of fluid retention.
