Heart failure in infants: signs and next steps

Babies cannot describe breathlessness, so caregivers may notice rapid or hard breathing, pauses, repeated coughing, or a baby who stops feeding to catch their breath. A baby may take a long time to finish feeds, sweat around the head while feeding, have fewer wet nappies, or gain weight slowly. Some infants have a fast heartbeat or look pale. These signs can have other causes, but heart failure is one condition clinicians may investigate, especially when several changes occur together.

Arrange prompt assessment with a paediatric clinician if feeding becomes unusually tiring, breathing is persistently faster, or growth is a concern. Record when symptoms occur, how long feeds take, wet-nappy changes, and any fever or illness. Bring the baby’s medicines and health records. Do not force a feed when the baby is struggling to breathe; keep the baby calm and follow the clinician’s feeding and medicine instructions.

Seek emergency help if the baby has severe breathing difficulty, blue or grey lips, pauses in breathing, is difficult to wake, becomes floppy, or cannot feed because of breathlessness. A sudden deterioration needs immediate care, even if a previous check was reassuring. Keep observing breathing and responsiveness while help is arranged.

Ask which signs should be monitored at home, how to support feeding safely, and whether the baby needs an EKG, echocardiogram, blood tests, or specialist review. Clarify medicine timing, follow-up, and who to contact if breathing or feeding changes.
