Heart failure and weight gain

When the heart’s pumping action is reduced, the kidneys may receive less blood flow and the body can hold on to salt and water. Fluid may collect in the legs, ankles, abdomen, or lungs. This can show up as a change on the scales before clothing feels tighter. Swollen ankles, cough, fatigue, or breathlessness may appear at the same time.

Not every increase reflects fluid. Food, constipation, reduced activity, and changes in clothing or weighing conditions can also affect the reading. A pattern over several days is more useful than one isolated measurement, especially when it occurs with swelling or a change in breathing.

If your care team has advised daily weighing, use the same scale at the same time, preferably after using the toilet and before breakfast. Record the result alongside swelling, breathing, cough, and activity tolerance. Follow your prescribed treatment plan and do not take extra diuretic medicine or change salt and fluid intake unless your clinician has given you a clear action plan.

Contact your heart-failure team or clinician when the pattern is rising, your shoes or rings become tight, or swelling is spreading. Mention missed medicines, salty meals, illness, and any new palpitations. A review may identify fluid retention, a rhythm problem, or another condition such as hypertension that needs attention.

Seek urgent medical help for severe breathlessness, breathlessness at rest, chest pain or pressure, fainting, confusion, or blue or grey lips. Emergency assessment is also needed if swelling and breathing difficulty worsen quickly. Seek prompt clinical advice for a continuing upward weight pattern with new cough, marked fatigue, swollen ankles, or reduced ability to carry out usual activities. Do not wait for the scales to change if you feel acutely unwell.

Ask which weight or symptom changes should trigger a call, how often to weigh yourself, and whether you have a written sick-day or diuretic plan. Clarify your recommended salt and fluid intake, what to do after a missed dose, and when a chronic disease review should take place.
