Heart failure and liver enzymes

The liver receives a large blood supply. When the heart cannot pump effectively, blood may back up in the veins leading to the liver, or the liver may receive less oxygen-rich blood. Either process can disturb enzymes measured in a blood test. The pattern and degree of change matter, and an abnormal result can have causes unrelated to heart failure, including medicines or another liver problem.

A clinician may compare the result with earlier tests and check bilirubin, albumin, clotting, kidney function and signs of fluid retention. Symptoms such as tiredness, swollen ankles, cough or worsening breathlessness help complete the picture, but liver enzymes alone do not measure how well the heart is working.

Arrange a review if a blood test has shown raised liver enzymes, particularly if you already have heart failure or your swelling and breathing have changed. Bring a list of prescribed medicines, supplements and recent illnesses. Do not stop heart or fluid medicines without medical advice; treatment changes may affect both circulation and blood results.

Follow the fluid, salt, weight-monitoring and medicine plan given by your heart team. A chronic disease review can help coordinate repeat blood tests and assess whether congestion is improving.

Seek emergency help for severe or rapidly worsening breathlessness, fainting, new confusion, blue or grey lips, or chest pressure that does not settle. Seek prompt medical advice for yellowing of the eyes or skin, very dark urine, marked abdominal swelling, vomiting blood or unusual bleeding. These signs need assessment and should not be attributed to heart failure without examination.

Ask which liver tests changed, whether the pattern fits congestion or reduced blood flow, and when they should be repeated. Clarify whether any medicine or supplement needs review, what changes in swelling or breathing should trigger a call, and how heart and liver results will be followed together.
