Headache while sleeping: what to know

Short answer
A headache that begins during sleep or is present on waking should be assessed by its timing, associated symptoms, and recurrence.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Record
- Waking time, pain pattern, snoring, and jaw tension
- Arrange review
- Repeated waking or progressive morning pain
Why it can happen
Jaw clenching or teeth grinding may cause morning pain around the temples, face, or jaw. An awkward pillow, neck position, irregular sleep, dehydration, or alcohol can also contribute. Snoring, gasping, witnessed pauses in breathing, and marked daytime sleepiness can point to a sleep-related breathing problem. Congestion from Allergic Rhinitis or Chronic Sinusitis may feel worse overnight. A headache that repeatedly wakes you, is new and progressive, or comes with vomiting or neurological changes needs medical review because the pattern may signal a problem beyond sleep habits.
What to try
Keep a regular sleep schedule, drink enough during the day, and note bedtime, waking time, pain location, snoring, jaw tension, and medicines. Check that your pillow supports a neutral neck position. Avoid alcohol if it seems linked to the pain. A dentist can assess tooth grinding, while a clinician can review sleep symptoms and other causes. Use pain medicine only as directed and seek advice if headaches are becoming frequent.
When to seek care
Get emergency help for a sudden extreme headache, weakness, confusion, seizure, fainting, trouble speaking, repeated vomiting, fever with a stiff neck, or new vision loss. Arrange prompt assessment if pain repeatedly wakes you, is worse each morning, follows an injury, or is accompanied by breathing pauses or severe daytime sleepiness.
Questions for your doctor
Could teeth grinding or sleep-related breathing be involved? Does my morning pattern need examination? Should someone observe my breathing during sleep? How can I avoid overusing pain medicine?
