Short answer

A headache that worsens while lying down deserves attention to its pattern, associated symptoms, and any warning signs.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

New positional pain with vomiting or visual change

What it may mean

Nasal or sinus congestion can feel more noticeable when you recline, especially with facial pressure, blocked breathing, or thick nasal discharge. Neck strain, an unsupportive pillow, teeth grinding, or a sleep position that tightens the scalp and neck can also contribute. Dehydration, heat exposure, viral illness, and anxiety may add to headache symptoms. A positional headache can occasionally reflect raised pressure inside the head, particularly when it is new, persistent, or accompanied by vomiting or visual change. The symptom alone cannot identify the cause.

What you can do now

Record when the pain starts, whether it improves on sitting or standing, its location and severity, and symptoms such as congestion, nausea, fever, or blurred vision. Rest in a quiet room, drink fluids if you can, and try gentle neck relaxation. Keep your head comfortably supported rather than sharply bent. Avoid repeatedly taking pain medicine beyond the label directions, and check with a pharmacist or clinician before using a new medicine if you have other conditions or take regular treatment.

When to seek care

Seek emergency help for a sudden severe headache, fainting, confusion, weakness, trouble speaking, a seizure, repeated vomiting, a stiff neck with fever, or new loss of vision. Arrange prompt medical assessment if the headache is steadily worsening, repeatedly wakes you, follows a head injury, or is clearly triggered by lying down and does not settle. A clinician may check your eyes, nerves, blood pressure, neck, and sinuses.

Questions for your doctor

Could my position, neck, sinuses, or medicines be contributing? What symptoms would make this urgent? Should my eyes, blood pressure, or neurological function be checked?

Questions people ask

Is a headache while lying down always serious?

No. Congestion and neck or sleep-position strain are possible, but a new or worsening positional pattern should be assessed if it persists.

What information helps a clinician?

Note the position that triggers it, the time of day, associated symptoms, recent illness or injury, and medicines used.
