Why position can matter

Lying flat can make nasal congestion feel heavier, so pressure around the face may be more noticeable with Allergic Rhinitis, a Common Cold, or Chronic Sinusitis. A pillow that bends the neck or strains the upper back can also cause a tension-type headache. Migraine may continue in any position and can include nausea or light sensitivity. Headache that is consistently worse when flat, especially if it is present on waking or paired with vision changes or vomiting, deserves clinical assessment because position-related pressure can have several causes. The pattern does not identify a diagnosis on its own.

Try these measures

Keep your head and neck in a comfortable neutral position rather than sharply flexed. If congestion is present, rest slightly propped up, drink fluids, and use only treatments suitable for you after checking with a pharmacist. Avoid alcohol and note sleep quality, snoring, recent illness, medicines, and the timing of pain. Do not repeatedly self-treat a positional headache without arranging a review, particularly when it interferes with sleep or daily activity.

When it is urgent

Call emergency services for a sudden severe headache, new weakness, confusion, fainting, seizure, trouble speaking, severe eye pain or vision loss, repeated vomiting, or a stiff neck with fever. Arrange prompt medical care when pain is new and progressively worse, regularly wakes you, is worse every time you lie flat, or follows a head injury. Marked thirst, dizziness, or weakness may point to Dehydration or Heat Exhaustion and should be addressed promptly.
