Headache when exercising: what to know

Short answer
Headache when exercising may follow exertion, heat, dehydration, missed food, or migraine, while a first sudden severe exercise headache needs urgent assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- First step
- Stop, cool down, and rest
- Record
- Effort, heat, fluids, food, and timing
- Urgent sign
- A first sudden severe headache during exertion
Possible explanations
Hard effort can widen blood vessels and tighten muscles in the neck and scalp, producing an exertion headache during or soon after activity. Heat, heavy sweating, inadequate fluids, or exercising without enough food can add to the strain. A migraine may be brought on by exertion and can include nausea or sensitivity to light. A respiratory infection, Common Cold, or COVID-19 may make normal exercise feel unusually demanding. Heat Exhaustion can cause headache with dizziness, weakness, heavy sweating, or nausea. These symptoms do not establish one diagnosis by themselves.
Steps to take
Stop exercising, move to a cool place, loosen tight clothing, and sip water. Do not restart while the headache, dizziness, nausea, or unusual weakness continues. Once well, return gradually with a gentle warm-up and avoid intense effort in extreme heat. Keep a record of the activity, intensity, temperature, fluids, food, sleep, and symptoms. Get individual advice before using pain medicine repeatedly, especially if you have kidney, stomach, heart, or blood-pressure problems.
Warning signs
Get emergency help for a thunderclap headache, collapse, confusion, weakness or numbness, trouble speaking, seizure, chest pain, severe breathlessness, repeated vomiting, or a stiff neck with fever. Seek prompt review for a first exercise headache, pain that peaks suddenly, headaches that recur with modest activity, or symptoms that do not settle with rest and fluids. Fever with severe body aches after outdoor exposure also warrants medical advice because Dengue Fever is possible in some settings.
What to ask
Ask whether you should pause particular workouts, how to hydrate safely in your conditions, whether your medicines or blood pressure contribute, and what exercise symptoms require emergency care.

Questions people ask
Can dehydration cause an exercise headache?
Yes. Fluid loss and heat can contribute, especially with thirst, dark urine, dizziness, or weakness, but severe symptoms need assessment.
When can I exercise again?
Wait until symptoms have fully settled and restart gently; ask a clinician for guidance if the headache was new, sudden, or recurrent.
