Headache so bad you cannot sleep: what to know

Short answer

A headache that prevents sleep needs attention to its pattern, associated symptoms, and any warning signs rather than a single assumed cause.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What it may mean

Pain can be driven by migraine, tension around the scalp and neck, poor sleep, missed meals, dehydration, heat, eye strain, or an infection. Nasal blockage and facial pressure can occur with Allergic Rhinitis or Chronic Sinusitis. Fever, cough, body aches, or recent exposure may point toward a viral illness such as Common Cold or COVID-19. Anxiety can tighten jaw and neck muscles and make pain harder to settle; Generalised Anxiety Disorder is one possible contributor, not a conclusion from a headache alone.

Notice whether the pain is throbbing or pressing, on one side or both, and whether light, sound, movement, nausea, fever, or neck stiffness accompanies it. A new pattern deserves more attention than a familiar, improving headache.

What you can do tonight

Rest in a quiet, dim room, sip fluids, and try a small meal if you have not eaten. Keep the neck supported and reduce screens, bright light, alcohol, and strenuous activity. If you use an over-the-counter pain medicine, follow its label and check with a pharmacist if you are pregnant, have kidney or liver disease, stomach ulcers, take blood thinners, or use other medicines. Avoid repeatedly taking pain medicine without advice, because frequent use can maintain headaches. Record when it began, what you took, and whether it helped.

When to seek care

Seek emergency care for a sudden explosive headache, new weakness or numbness, confusion, fainting, seizure, trouble speaking or seeing, repeated vomiting, a high fever with stiff neck, or a headache after a significant head injury. Get prompt medical advice for a new severe headache, pain that repeatedly wakes you, worsening symptoms, persistent fever, or headache with marked dehydration. Heat Exhaustion and Dengue Fever can require assessment when headache occurs with heat exposure, fever, rash, bleeding, or unusual weakness.

Questions for your doctor

Ask: What features of my headache suggest migraine, infection, dehydration, or another cause? Do I need an examination or testing? Which pain medicine is suitable with my health conditions and current medicines? What pattern should I record, and when should I return if sleep remains disrupted?

Questions people ask

Should I ignore a headache that wakes me?

No. Arrange medical advice when a headache is new, severe, recurrently wakes you, or is worsening; emergency symptoms need immediate care.

Can dehydration cause a nighttime headache?

Yes. Low fluid intake, heat, sweating, vomiting, or diarrhoea may contribute, especially with thirst, dry mouth, or dark urine.
