Headache behind the right eye: what to know

Headache behind the right eye should be assessed by its onset, severity, vision changes, and symptoms affecting the eye or nervous system. One-sided pain can be mild or disabling, and the eye location does not by itself reveal the cause.

Migraine may cause one-sided throbbing pain with nausea or light sensitivity. Cluster-type headache can be focused around one eye and may cause tearing, nasal blockage, or marked restlessness. Sinus pressure may occur with Allergic Rhinitis, Chronic Sinusitis, Common Cold, or COVID-19, particularly when congestion accompanies the pain. Long periods of focusing, dry eyes, a change in vision, jaw clenching, and neck tension are other possibilities. Dehydration or Heat Exhaustion may be relevant after sweating, heat exposure, dizziness, or unusual tiredness.

Stop driving if your vision feels altered. Rest away from bright light, take screen breaks, drink fluids, and eat if you have skipped food. A cool compress can be placed over the closed eye without pressure. Write down when the pain began, how quickly it peaked, whether eye movement changes it, and whether the eye is red or watery. Follow the label for any familiar pain medicine and ask a pharmacist about safety with your medicines, pregnancy, allergies, or chronic illness. Do not put drops or other products in the eye unless they are intended for you.

Get emergency help for sudden or unusually severe pain, sudden loss or major change of vision, a red painful eye, halos with nausea, new weakness, confusion, difficulty speaking, fainting, fever with a stiff neck, or a head injury. Seek medical review for repeated episodes, a new pattern, pain that persists, or symptoms that interfere with sleep or normal activity. An examination can separate eye problems from headache, sinus, muscle, or nerve causes.
