Hand eczema and cracked fingers: what to know

Hand eczema is also called hand dermatitis. It may affect the fingertips, finger joints, palms, or the spaces between fingers. Redness, scaling, burning, itching, and small blisters can occur before the skin splits. A crack is a break in the skin barrier; it can sting when exposed to water, soap, or cleaning products and may bleed lightly when the area is stretched.

Cracking does not identify one single trigger. Frequent wet work, detergents, solvents, friction, cold air, and contact allergy can all aggravate hand eczema. Other skin conditions can look similar, so a clinician may examine the pattern and ask about work, hobbies, gloves, and new products.

Rinse with lukewarm water, use a gentle fragrance-free cleanser, and pat rather than rub. Apply a thick emollient after washing and whenever the skin feels tight; emollient therapy helps replace some of the barrier protection lost through dryness. At night, a generous layer with soft cotton gloves may reduce friction while you sleep.

Use protective gloves for cleaning or other wet tasks, but take breaks so sweat does not remain trapped. Keep fingernails short and avoid picking loose skin. A clinician may prescribe a topical corticosteroid for an inflamed flare, with instructions about where and how long to use it. Persistent or extensive eczema may prompt discussion of narrowband UVB phototherapy.

Arrange a medical assessment if cracks keep reopening, interfere with work or sleep, or do not settle with careful skin protection. Seek prompt care for spreading redness, increasing warmth or swelling, pus, fever, or rapidly worsening pain, because broken skin can become infected. Sudden swelling of the lips or breathing difficulty needs emergency help.
