What should you know about hamstring strain pain?

Short answer

Hamstring strain pain often follows a sudden stretch or forceful contraction and improves with protection, gradual movement, and rehabilitation.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What the pain can mean

The hamstrings are muscles at the back of the thigh. A strain occurs when muscle fibres are overloaded, sometimes during sprinting, jumping, kicking, or an awkward step. Pain may be sharp at the moment of injury, then become sore or tight with walking, bending the knee, or lengthening the leg. Bruising, swelling, weakness, or a tender spot can occur, especially with a more substantial injury. Thigh pain can have other causes, so the location and movement pattern matter when a clinician assesses you.

Pain that eases at rest but returns when you accelerate or stretch is a reason to progress carefully rather than test the muscle repeatedly.

What to do now

For the early phase, reduce sprinting, deep stretching, and activities that reproduce the pain. Use comfortable walking as tolerated, and consider cold therapy wrapped in a cloth for short periods if it feels soothing. Avoid forceful massage directly over a fresh bruise. As pain settles, a physiotherapist can guide gentle range-of-motion work, followed by strengthening that gradually loads the hamstring and hip. Exercise Therapy can help structure this progression. A Return-to-Sport Programme is useful when running, cutting, or sport-specific drills are part of your routine.

When to seek care

Arrange an assessment if you cannot walk normally, felt a snap, developed marked bruising or swelling, or have clear weakness. Seek prompt medical attention for severe pain, numbness, a cold or pale foot, or rapidly increasing swelling. An examination is also worthwhile when symptoms are not steadily improving or keep returning after activity.

Questions for your doctor or physiotherapist

Ask which movements should be paused, what level of walking is appropriate, and how strength will be tested before running. You can also ask whether imaging is needed, how to reduce reinjury risk, and when a staged return to sport is appropriate.

Questions people ask

Should I stretch a painful hamstring?

Do not force a painful stretch early on. Gentle movement within comfort is usually a better starting point, followed by guided flexibility work as the muscle settles.

Can I keep exercising?

Choose activity that does not increase pain during the session or leave it worse afterward. A clinician can help substitute safe upper-body or low-load exercise.
