Why a hamstring may cramp

A cramp can follow strenuous exercise, prolonged sitting, muscle fatigue, or a change in training. Heat, fluid loss, and some medicines can also contribute. The muscle may feel hard and knotted, with pain that settles after the spasm but leaves soreness. Repeated cramps are not automatically a sign of another disease, yet a clinician may consider dehydration, thyroid problems, circulation issues, kidney disease, low vitamin D, reduced muscle mass, or a muscle strain when the pattern is persistent or unusual.

Replace fluids after sweating and return to exercise gradually, with a warm-up and regular breaks. Avoid forceful stretching during the spasm or intense training while the area remains sore. Record what you were doing, how long the episode lasted, and any medicines or new exercise routines; this helps distinguish ordinary cramps from injury.

Arrange medical review if cramps are frequent, severe, new after a medicine change, disturb sleep, or do not settle with simple measures. Seek urgent help for a swollen, red, hot leg; sudden weakness or numbness; inability to walk; severe pain after an injury; or chest pain and breathlessness.
