Biceps femoris hamstring strain: what should you know?

How the structures involved in Hamstring Strain differ from normal

Short answer

A biceps femoris strain is an injury to the outer hamstring muscle, usually managed with graded rehabilitation and a careful return to loading.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Muscle involved
Biceps femoris, on the outer side of the hamstring group
Common triggers
Sprint effort, acceleration, or sudden direction change
Recovery focus
Progressive strength and sport-specific loading

Understanding the injury

The biceps femoris is one of the hamstring muscles at the back of the thigh. It helps bend the knee and extend the hip, particularly during running, sprinting, or changing direction. A strain occurs when muscle fibres are overloaded and irritated or torn. Pain may be felt in the back or outer thigh, with tenderness, weakness, bruising, or discomfort when stretching the leg.

The location and severity matter. Pain near the sitting bone, knee, or outer thigh can have different causes, so an examination is useful when symptoms are significant or do not settle. Thigh pain that changes your walking pattern should not be tested repeatedly with sprinting.

Supporting recovery

During the early phase, reduce sprinting, deep stretching, and movements that reproduce pain. Gentle walking within comfort and simple range-of-motion work may be suitable, followed by progressively harder strengthening. Exercise therapy can target hamstring capacity, hip control, and the coordination needed for your sport. Increase one demand at a time and use next-day symptoms as feedback.

Sports physiotherapy can assess strength differences, running mechanics, and the point at which faster movement is appropriate. A return-to-sport programme should include jogging, acceleration, deceleration, and sport-specific drills rather than relying only on pain at rest. PRP injection is a treatment discussion for selected persistent injuries, not an automatic first step.

When to seek assessment

Arrange care if you felt a sudden pop, developed extensive bruising, cannot walk normally, or have marked weakness. Seek prompt attention for severe swelling, numbness, a cold or pale foot, or rapidly increasing pain. A clinician may examine strength and tenderness and decide whether imaging is useful.

Return for review if symptoms plateau, recur when speed increases, or prevent ordinary activity. Rushing back while the muscle remains weak can expose it to another overload.

Questions for your clinician

Ask: Which part of the hamstring is involved? What loading is safe this week? When can I begin running? Which strength test should I pass before sport? Would a rehabilitation programme or another treatment be appropriate for my injury?

Questions people ask

Where does a biceps femoris strain hurt?

It can cause pain and tenderness along the back or outer thigh, sometimes closer to the hip or knee.

When can I return to sport?

Return is guided by restored strength, controlled running, and sport-specific tasks without a concerning symptom response.
