What one-sided pain can tell you

The familiar growing-pain pattern is usually an ache in both legs that comes and goes, often later in the day, without daytime limping or visible changes. One-sided pain may follow a knock, a new sport, tight footwear, or a muscle strain. Pain focused below the kneecap in an active child can occur with Osgood-Schlatter Disease, while heel pain in an older child can occur with Sever's Disease. These conditions do not make every one-sided ache a diagnosis; the exact site and examination matter.

What to do at home

Ask the child to point with one finger to the most painful spot and watch whether they walk, climb, and play normally. Pause the activity that provokes pain and choose easy movement instead. Check for bruising, swelling, warmth, or a tender area, and write down whether symptoms appear during activity, at rest, or overnight. Use a wrapped cold pack for a recent minor knock if comfortable. Arrange assessment when pain persists, repeats in the same place, or changes normal movement.

When to seek care

Seek prompt care if the child limps, refuses to stand, has swelling or warmth, develops fever, or cannot use the leg normally. Urgent assessment is needed after a significant injury, with severe or rapidly increasing pain, or when the leg appears misshapen. Immediate help is appropriate for a very unwell child or sudden one-sided swelling with breathing symptoms.
