The usual pattern

The ache is commonly felt in both legs, especially in the calves, thighs, or behind the knees. It may appear after an active day and be absent by morning. Growing pains do not usually cause a swollen joint, rash, fever, weakness, or a change in walking. The term describes a pattern; it does not establish that growth itself is the cause.

Pain in the front of the knee near its growth area can occur with Osgood-Schlatter Disease. Pain at the heel during running or jumping can occur with Sever's Disease. A focused history and examination help separate these from night-time muscle aches.

Managing an episode

Offer reassurance while checking the exact location and both legs for visible swelling or skin changes. A warm bath, wrapped warm pack, or gentle massage may make the muscles more comfortable. Slow stretching can be tried if it feels easy. Keep a note of timing, activity, sleep disruption, and response to comfort measures so a clinician can see the pattern.

Keep activity comfortable and avoid pushing through sharp pain. If medication is considered, follow the product instructions and obtain advice from a pharmacist or clinician when there are allergies, other medicines, or chronic health conditions.

Warning signs

Seek medical advice when pain is persistent, one-sided, present in the daytime, or associated with a limp or reduced activity. Prompt assessment is appropriate for fever, joint warmth, redness, swelling, unusual tiredness, weakness, numbness, or unexplained weight change. Get urgent help for severe pain after injury or inability to stand or walk.
