What should you know about groin buttock pain?

Short answer
Groin and buttock pain can come from the hip, pelvic joints, muscles or irritated nerves, so the pattern of pain and accompanying symptoms guide assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Possible regions
- Hip, pelvis, buttock or lower back
- Useful clues
- Sitting, stairs, hip rotation and leg symptoms
- Urgent nerve signs
- New bladder, bowel or saddle-area changes
What the pattern may suggest
Groin pain often points clinicians toward the hip or nearby adductor muscles, while buttock pain may arise from the gluteal tendons, sacroiliac region or tissues around the sciatic nerve. Pain that travels down the leg, tingling or weakness may indicate nerve irritation. Pain with sitting, stairs, running, hip rotation or lying on one side gives additional clues, but no single pattern confirms a diagnosis.
A physical examination can compare hip movement, spinal motion, pelvic control and strength. The assessment may also check for symptoms outside the musculoskeletal system when the history suggests them.
Useful first steps
Keep activity gentle and varied rather than staying in one posture for a long time. Reduce hills, deep squats, heavy lifting or prolonged sitting if they clearly worsen symptoms. Short walks and comfortable position changes may be better tolerated. Avoid aggressive stretching of the buttock or groin until the cause is clearer. A physiotherapist can select exercises for hip control, trunk support or nerve sensitivity based on the examination.
When to seek care
Arrange medical review when pain persists, disrupts sleep, repeatedly returns or is accompanied by leg weakness, altered sensation or difficulty walking. Seek urgent help for new loss of bladder or bowel control, numbness around the saddle area, rapidly increasing weakness, fever, or severe pain after a fall.
Questions for your doctor
Ask whether the symptoms fit the hip, sacroiliac joint, gluteal muscles or a nerve; which activities to pause; whether an examination of the back is needed; and which changes should prompt urgent review.

Questions people ask
Does groin and buttock pain always mean sciatica?
No. Hip, pelvic-joint and muscle problems can also cause this combination; travelling pain, tingling or weakness make nerve involvement more concerning.
Should you rest completely with this pain?
Complete rest can increase stiffness. Gentle movement within tolerance is often preferable, while aggravating activities are reduced until assessment.
