What tophi mean

Tophi are firm collections of urate crystals in soft tissue, often near the toes, fingers, elbows, ankles or other joints. They can develop when gout remains active for a long period. A tophus may be painless between flares, yet it can restrict movement, press on footwear or become tender and inflamed. The presence of a lump does not by itself show how severe the joint damage is; examination and sometimes imaging help clarify its effect. Gout can also coexist with chronic kidney disease or obesity, so the wider health picture matters when choosing medicines.

Treatment options

A clinician may prescribe long-term urate-lowering treatment and adjust it using symptoms, examination and blood results. Acute inflammation is managed separately, with a suitable anti-inflammatory plan chosen around kidney function, stomach health and other medicines. Do not squeeze, puncture or try to drain a tophus at home. Comfortable, roomy footwear and protection from rubbing can reduce skin injury. If a deposit interferes with walking, joint use or wound healing despite medical treatment, a specialist may discuss removal; surgery is selective because healing and crystal control still need attention.

When to seek care

Arrange prompt medical assessment for a tophus that becomes rapidly more painful, hot, red, swollen, leaks fluid or develops a wound. Fever or feeling acutely unwell with a hot joint needs urgent assessment because infection can resemble a gout flare. Seek help sooner if swelling prevents weight-bearing, a toe changes colour, or pain is not settling with the plan provided. New joint pain or swelling should be assessed rather than assumed to be gout.
