Why the pain can pulse

A gout flare causes inflammation in and around a joint. Increased sensitivity, swelling, and pressure can make pain feel pulsing or throbbing, and even light contact may be uncomfortable. The big toe is common, while the ankle, knee, or midfoot can also be involved. Throbbing pain by itself does not prove gout: injury, infection, other arthritis, or a skin problem can feel similar. A clinician may use the timing, appearance, examination, health history, and tests such as joint-fluid analysis to clarify the cause.

Ways to reduce strain

Rest the painful joint and elevate the limb when possible. A cool pack wrapped in fabric may reduce discomfort; do not apply ice directly to skin. Wear loose footwear if the foot is involved and avoid pressing on the joint. Ask a clinician or pharmacist about appropriate treatment rather than taking leftover medication. Some anti-inflammatory options are unsuitable with kidney, stomach, heart, or blood-pressure problems. If gout has already been diagnosed, follow your flare plan and keep taking longer-term medicine as prescribed unless your clinician tells you otherwise.

When throbbing pain is urgent

Seek urgent assessment for a hot, swollen joint with fever, chills, rapidly spreading redness, a wound, pus, severe illness, or inability to use the limb. Infection can produce pain and swelling that resemble gout. Arrange a review if this is your first episode, pain is worsening, symptoms persist, or attacks return. A painful ankle or knee may need a different evaluation from a simple foot flare.
