Gout pain level: what severe pain may mean

Urate crystals activate inflammation inside a joint. That response can make the joint feel throbbing, burning, or sharply tender, sometimes making ordinary contact difficult. Pain may peak quickly and be accompanied by red or shiny skin and swelling. The pain score does not prove that a flare is gout, and milder pain does not rule it out. A clinician considers the location, appearance, timing, medical history, and sometimes test results. A painful, hot joint can also result from infection or injury, both of which need different care.

Stop activities that load the joint and support it in a comfortable position. Elevation and a cool pack wrapped in cloth may reduce discomfort for brief intervals. Take only the treatment recommended for you and check before using an over-the-counter anti-inflammatory medicine if you have kidney disease, stomach ulcers, heart problems, are pregnant, or take blood thinners. Note the time pain started, how quickly it changed, and whether swelling or fever appeared; these details help a clinician judge the next step.

Seek urgent care for severe joint pain with fever, chills, confusion, feeling very unwell, or rapidly spreading redness. Get prompt assessment when the pain is new, follows trauma, prevents weight-bearing, or does not improve as expected. Do not assume that a high pain level is simply a gout flare, because infection can damage a joint without timely treatment.
