What the pain location can tell you
Gout happens when urate crystals collect inside a joint and trigger inflammation. The area may become suddenly very tender, warm, red, and swollen. A flare can start in the big toe, but gout can also involve the midfoot, ankle, knee, wrist, fingers, or elbow. Pain may make even light contact or movement uncomfortable. Having pain in one of these areas does not by itself confirm gout; infection, injury, osteoarthritis, and other inflammatory conditions can look similar. A clinician may use the examination and, when needed, joint fluid or blood testing to clarify the cause.
What you can do during a flare
Rest the affected joint and raise it when practical. A cool pack wrapped in cloth may soothe discomfort for short periods; avoid placing ice directly on skin. Keep pressure off a painful foot or knee and choose loose, supportive footwear. Do not start, stop, or change gout medicine without medical advice, particularly if you have kidney disease, stomach problems, or take medicines that affect bleeding. Arrange a clinical review if episodes recur, move to new joints, or interfere with walking.
When a painful joint needs prompt care
Seek urgent medical assessment for a hot, swollen joint with fever, chills, feeling faint, or rapidly worsening pain. A joint infection can resemble a gout flare and needs different treatment. Get prompt advice when this is your first episode, the pain follows an injury, you cannot bear weight, or symptoms are not settling with the plan given to you.

Yes. Gout can inflame the knee as well as foot, ankle, wrist, finger, or elbow joints. A hot swollen knee still needs assessment because infection and injury can cause similar symptoms.
