Understanding toe gout

Gout is a crystal-related inflammatory arthritis. It often involves a toe joint, but the affected toe can vary and more than one joint may hurt. During a flare, swelling and sensitivity may make socks, shoes and walking uncomfortable. The symptoms can resemble a sprain, cellulitis or another inflammatory arthritis, particularly when there has been an injury or skin break. Diagnosis may involve an examination, medical history and tests chosen by the clinician; a urate blood result by itself does not always settle the question.

Steps during a flare

Rest the foot, raise it when sitting and use loose footwear or open sandals if safe. Place a wrapped cool pack over the area for brief intervals. Avoid rubbing the joint or forcing it into a shoe. Take prescribed flare treatment exactly as directed, and check before using anti-inflammatory medicines if you have kidney, stomach, heart or pregnancy concerns. Once the episode improves, review long-term prevention, medication adherence, hydration advice and any personal patterns with your clinician.

When to get medical help

Get prompt medical advice for a first flare, a painful wound, rapidly spreading redness, fever, chills or inability to bear weight. An infected joint or skin infection needs different treatment from gout. Urgent assessment is also needed after significant trauma, with severe unremitting pain, or when you feel faint or very unwell. Repeated attacks deserve a prevention discussion even if they settle between episodes.
