Gout in the hip joint: what to know

Gout is an inflammatory arthritis caused by urate crystals. The hip is surrounded by deep tissues, so a flare there may feel like pain in the groin, buttock, or upper thigh rather than a visibly swollen joint. Hip gout is less straightforward to identify from symptoms alone because infection, osteoarthritis, bursitis, and other problems can produce overlapping pain. A clinician may consider your pattern of attacks, examination findings, urate results, imaging, and sometimes fluid or tissue testing. Chronic kidney disease and obesity may be part of the medical history, but neither proves that hip pain is gout.

Reduce activities that aggravate the hip and use support if walking feels unstable. A cool pack wrapped in cloth may ease discomfort for brief periods; do not place ice directly on skin. Book a medical review for new deep hip pain, especially if movement is restricted or a previous gout diagnosis has not been confirmed. Prescribed flare treatment and urate-lowering treatment serve different purposes, so follow the plan given for you. Share your full medicine list and any kidney or stomach problems before taking anti-inflammatory medicine.

Seek urgent care for severe hip pain with fever, chills, confusion, rapidly worsening symptoms, or inability to stand or bear weight. These features can signal infection or another urgent problem and should not be attributed to gout without assessment. Prompt review is also sensible after a fall, with a new leg-length concern, or when pain continues despite the agreed treatment.

Ask what other hip conditions need excluding, whether imaging or fluid testing would help, which medicines are safe for you, and whether a long-term urate plan is appropriate.
