What should you know about gout in back?

Gout is caused by urate crystal inflammation, usually within a joint. Back pain has many more common explanations, including muscle strain, disc irritation, arthritis, and kidney or abdominal problems. In someone with established gout, crystal inflammation can occasionally affect joints near the spine, but the location and pattern need assessment. Pain may be deep, localised, and worse with movement, yet those features are not specific. A clinician will ask about fever, injury, urinary symptoms, leg symptoms, previous gout attacks, kidney disease, and medicines. Examination and, when appropriate, imaging or other tests help separate spinal, joint, nerve, kidney, and infectious causes.

Avoid heavy lifting and movements that sharply increase pain, but change position gently rather than staying completely still for long periods. Use a wrapped cool or warm pack according to what feels more comfortable, and take only medicines that are safe for you and recommended by a clinician or pharmacist. Do not assume a back flare is gout or start leftover medication. Tell the healthcare professional about kidney disease, stomach problems, blood thinners, and any change in urine. A confirmed gout diagnosis may lead to a broader prevention plan and review of urate-lowering treatment.

Seek urgent care for back pain with fever, chills, confusion, or feeling very unwell. Go promptly for new leg weakness, numbness around the groin, loss of bladder or bowel control, or difficulty passing urine. Sudden severe pain after injury, blood in the urine, or pain with vomiting also needs prompt assessment. Make an appointment for persistent, recurrent, or unexplained pain, particularly if you have gout or kidney disease.
