Why the toe reacts

Urate crystals may collect in the joint at the base of the big toe or in another toe joint. The immune response around them causes pain and swelling that can start quickly, sometimes making socks, shoes, or a bedsheet feel uncomfortable. Ankle or knee symptoms can occur in gout too.

Toe redness and pain do not confirm gout on their own. Injury, skin infection, a bacterial joint infection, and other forms of arthritis can produce similar signs. A clinician may examine the toe and consider joint-fluid testing or other tests when the picture is unclear.

Caring for the toe

Red flags around a painful toe

Seek urgent care for a hot swollen toe with fever, chills, rapidly spreading redness, pus, or feeling very unwell. Prompt assessment is also appropriate for a first attack, severe pain after an injury, loss of sensation, or inability to walk. Infection and injury need different treatment from gout.
